See the yellow box in the image. You can see the inner and outer rings are aligned in this image which is NOT what you want. Those need to be offset.
Anything in the range of 3 inches up to 4 inches will work fine, 3.5" would be ok
I would imagine if you take it apart that you should see some wear on the shaft if that is the case

Are you running the AVHC in AUTO mode or MANUAL?
